We are proud to announce that Cornwall Hospice Care are now recognised as being Homeless Friendly. Their pledge includes Mount Edgcumbe Hospice and St Julia’s Hospice, The Out Patients Service, In Hospital support plus their Community Support Service which offers 24 hour/7 day specialist advice service for healthcare teams throughout Cornwall to support the care of patients, wherever they are. Patients are referred by the Palliative Care teams or any of the key healthcare professionals who are involved in the person’s care, but always in liaison with their GP or hospital consultant so that we can ensure continuity and coordination of care. This forward-thinking organisation approached Homeless Friendly due to wanting to ensure that they offer appropriate support to people who are homeless. We have been able to collate local information on their behalf and been informed that they have already made use of the duty to refer. This organisation clearly go the extra mile for the people that they support, including being a key player in the local bereavement cafes.
